What is actually causing your back pain

Most people who walk in here have already decided what is wrong with them. Usually it is a disc, because that is the word that comes up first when you search your symptoms at eleven at night. Occasionally they are right. More often, in the cases we see, the pain is coming from how the body has been loading and moving for months or years, and the back is simply where it finally showed up.

Muscles that should be doing work stop doing it. Others take over and get overworked and irritable. Joints that should move freely stiffen, and the ones above and below them move more to compensate. None of this is dramatic on a scan. All of it hurts.

This matters because it changes what the treatment is for. If the problem is structural, you need imaging and a specialist, and we will tell you so. If the problem is how you load and move, then rubbing the sore bit will help for about three days and then you are back where you started. That gap between short-term relief and the thing actually changing is where most back pain treatment in Bangkok quietly fails people.

There is also a boring answer that nobody wants: a lot of back pain settles on its own within a few weeks regardless of what anyone does to it. If you tweaked your back on Saturday and it is already easing, you may not need us at all. We would rather say that than sell you a package.

The words people arrive with

Sciatica is not a diagnosis, it is a description. It means pain travelling down the leg along the path of the sciatic nerve, and several different things can produce it. Some of them are a disc pressing on a nerve root; others are a nerve being irritated further down its course by tissue that has got tight and angry around it. They feel similar to the person experiencing them and they need different treatment, which is why the assessment matters more than the label.

A herniated or bulging disc is a real thing and it is also one of the most over-attributed findings in medicine. Discs change shape as people get older in much the same way hair goes grey, and plenty of people walking around without a twinge would show something on a scan if you put them in one. A disc finding on your imaging is information, not necessarily the explanation. What matters is whether the symptoms match the finding.

Lower back pain is the bulk of what we see, usually a mix of stiffness through the hips and mid-back with the lumbar spine taking the load that those two should have shared. Upper back and neck pain tends to arrive from the same desk, and is often treated separately when it is the same problem expressed higher up.

Post-operative rehabilitation is a different job again. If you have had spinal surgery, the surgeon sets the constraints and we work inside them. Bring the operation notes and whatever the surgeon told you about restrictions and timelines.

When to skip us and see a doctor

Some back pain is a medical problem, not a movement problem, and the sensible thing is to go to a hospital rather than a physiotherapy clinic. Go and get seen the same day if any of the following apply.

  • You have lost control of your bladder or bowels, or you have numbness around the groin, buttocks or inner thighs.
  • You have weakness in a leg or foot that is getting worse, not just pins and needles.
  • The pain started after a serious fall, a crash, or any significant trauma.
  • You have a fever, unexplained weight loss, or a history of cancer alongside new back pain.
  • The pain is severe at night, does not settle whatever position you lie in, and is not related to movement.

None of that is common, and listing it is not an attempt to alarm you. It is there because a physiotherapy clinic that does not tell you when to go elsewhere is not being straight with you. Aspire assesses and treats; we do not diagnose disease, and we do not replace your doctor.

What happens in the first session

The first consultation is free and it is mostly talking and watching. We want the history properly, not the two-line version: when it started, what makes it worse, what it stops you doing, what has already been tried and what happened. The sentence that usually tells us the most is the one about what you can no longer do without thinking about your back.

Then we watch you move. Bending, rotating, loading one leg, getting off a chair, walking. We test where movement is genuinely restricted and where it is being guarded, and we test what is weak against what is only painful. Weak and painful are different problems with different answers, and they are easy to confuse from the inside.

You leave that session knowing what we think is going on, what we would do about it, roughly how long it should take, and what would make us send you to a doctor instead. If the answer is that you do not need treatment, that is what you get told.

What treatment looks like

Hands-on work does the early part. Joint and spine mobilisation, deep tissue and sports massage, trigger point therapy and myofascial release all have a role, and which of them you get depends on what the assessment found rather than on what we happen to like doing. The honest description of this phase is that it buys you room to move. It calms things down and restores range so that the next part is possible.

The next part is loading. Once you can move without the system guarding, the muscles that stopped working have to be taught to work again, and then loaded until they are strong enough to hold up under a normal life. This is the phase people skip, because by then the pain has gone and the motivation goes with it. It is also the only phase that changes whether you are back here in eight months.

You will get things to do between sessions. Not forty minutes of exercises you will abandon by Thursday, but a small number of specific ones that matter, with a clear reason for each. If we give you something you cannot realistically fit into your week, tell us, and we will give you something you can.

The part most clinics skip

A physiotherapy clinic discharges you when the pain has gone. That is a reasonable place for a clinic to stop, and it is also the point at which most people go back to exactly the life that produced the problem. The back is quiet, nothing underneath it has changed, and it recurs.

Office syndrome and the Bangkok desk problem

A large share of the back pain we see in Bangkok is not an injury at all. It is nine hours at a desk, a commute that adds another ninety minutes of sitting, air conditioning that keeps everything cold and stiff, and a gym habit that got quietly dropped somewhere around the second year here. Office syndrome is the local shorthand for it and it is a real pattern, even if the name is vague.

The frustrating thing about desk-related back pain is that the fix is unglamorous. It is rarely one perfect chair or one stretch. It is breaking up the total sitting load, getting some rotation and extension back into a spine that spends all day flexed, and building enough strength that eight hours of static posture is no longer near the limit of what your body can tolerate. What it costs, and insurance

Most international health policies held by expats in Thailand cover physiotherapy, usually with a referral and sometimes with a session cap. We are not going to pretend to know the terms of your specific policy, but we supply the documentation providers ask for, and it is worth a five-minute call to your insurer before you start rather than after.

Your physiotherapists

You will be treated by Boom Namsak, Gino Udomthanakij or Som Benjaprasertsri. All three are licensed physiotherapists working in English and Thai. Boom also works as a strength coach, which is why he tends to pick up the cases where the rehabilitation needs to carry on into proper training.

You see the same physiotherapist each time. Continuity matters more than it sounds like it should, because the useful information in a rehabilitation case is the change between sessions, and that is hard to read if a different person is looking each week. You can see the full team here.

Us or a hospital?

Bangkok has excellent hospitals and there are cases where one of them is plainly the right answer. If you need imaging, a specialist opinion, an injection or surgery, a hospital does that and we do not. If you have red flag symptoms, go there first.

What a hospital rehabilitation department is less good at is the long tail. Appointments are short, you rarely see the same therapist twice, and the discharge point is when the pain has gone rather than when you are robust again. That last stretch, from not hurting to actually strong, is what a private studio with strength coaches on site is built for.

The sensible version for a lot of people is both. Get the diagnosis where diagnosis is done properly, then do the rehabilitation somewhere that will still be working with you three months later.

Common questions

Do I need a referral or a scan before I come?

No. You can book the free consultation directly. If we think you need imaging or a specialist opinion, we will tell you in that session and say why.

How long until the pain goes?

It depends on what is causing it and how long it has been there, and anyone who gives you a number before assessing you is guessing. What we will do is give you an estimate after the first session and tell you if we are not on track.

Will you just give me a massage?

Hands-on work is part of it, not the whole of it. If manual therapy alone were enough, the problem would have stayed fixed the last time somebody worked on your back.

Can I keep training while I am being treated?

Usually yes, with modifications. Stopping everything is rarely the right call and often makes the recovery slower. We will tell you what to change rather than what to stop.

Is it covered by insurance?

Physiotherapy is covered under most international policies, often subject to a referral or a session limit. We provide the paperwork; check your own terms with your provider first.

Do you speak English?

Yes. The clinic works in English and Thai, and a good part of our client base is the Sukhumvit expat community.
